How do I use a Purchase Order as my form of payment?

If you haven’t already established an account with us, you'll first need to print out this form and fax it to the Bulk Sales Department at 859-514-6814. Once your account is set up, you simply fill out our order form and choose the Purchase Order payment option.

For privately funded Agencies/Companies, use this Credit Application.

For Government/State Funded Agencies, use this Credit Application.
