I need to use a purchase order as my form of payment. How do I do this?
If you haven’t already filled out a Credit Application with us, you'll first need to print out this form and fax it to the Bulk Sales Department at 859-514-6814. Once your credit is approved, you simply fill out our order form and choose the Purchase Order payment option.
