David Shaffer, MB, FRCP, FRCPsych, is Irving Philips Professor of Psychiatry at Columbia University College of Physicians and Surgeons, New York, and at the New York State Psychiatric Institute.

Christopher P. Lucas, MB, ChB, MMedSc, MRCPsych, is Assistant Professor of Psychiatry at Columbia University College of Physicians and Surgeons, New York, and at the New York State Psychiatric Institute.

John E. Richters, PhD, is a developmental psychologist at the National Institute of Mental Health, where he has served as Chief of the Disruptive Disorders Program and Assistant Chief of the Child and Adolescent Disorders Research Branch.
